1. Have you completed the medical/registration form for the day camps? Forms may be picked up at the Recreation Office, Town Hall and the No. Castle Community Center.
a. Complete the immunization record using exact dates (month, day, year).
b. Do not attach the doctor’s record – copy the dates onto our form
c. Don’t forget to sign and date the form.
2. Have you included your check or checks?
a. Checks should be made payable to North Castle Recreation.
b. Use separate checks for each child. Combination checks will be returned, and a space will not be held for registrants.
c. If you have campers in different camps, please use CHIPPEWA as the FIRST CHILD RATE.
3. Have you included a copy of your child’s birth certificate for Camps Pic-A-Bo and Kick-A-Poo.
4. Make sure all copies of the form are legible. Press hard with pen or type.
5. Please remember that you are limited to one placement request only! No transfers will be made after the registration is received.
6. Have you used the regular registration forms for the specialty camps and adult programs.
a. Please fill out the sports camp medical release form for all sports camps and return with your registration card and your check made payable to North Castle Recreation and send to 40 Maple Avenue, NY 10504.
